Community Services Group (CSG) is recruiting for a Health Services Coordinator for our Adult Mental Health Services in York and Lancaster Counties.  We are looking for an experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) for this position.

The Health Services Coordinator will be responsible for coordinating, monitoring, and providing nursing services and oversight for our Transition to Community Program (TTC) in Lancaster, Long Term Structured Residence (LTSR) Program in Spring Grove and Enhanced Community Residential Rehabilitation (ECRR) Program in York.

This is a full-time position (40 hr/wk) with mainly Monday to Friday hours.  2 days per week in Lancaster County and 3 days per week in York County.  Must be able to work flexible hours to meet individual's and program's needs.

Essential Duties and Responsibilities:

  • Assesses the individuals' health needs through observation of behavioral changes and physical signs and symptoms.
  • Acts as the liaison with CSG, physicians, pharmacists, and other medical professionals.
  • Provides on-going training to staff in medications, medical regulations, terminology, and other health related information.
  • Monitors vitals, intakes, and outputs.
  • Reviews medical records.
  • Travels to homes, offices, programs, hospitals, etc.
  • Oversees implementation and follow through of medical care plans.
  • Participates in scheduled treatment team meetings and interdisciplinary evaluations of individuals.
  • Ensures that medical supplies are purchased and stocked as needed.
  • Prepares required daily, monthly, quarterly, and annual reports.
  • Develops and supervises written treatment plans or nursing care plans for individuals in conjunction with the treatment team.
  • Supervises, schedules, and evaluates Nurses if assigned supervisory responsibility of others.
  • Administers medications, provides treatments, and other health services as prescribed by the physician and in accordance with state regulations.
  • Confirms proper nursing coverage is present as needed.
  • Determines primary support and coverage for the prescribers and ensures that each nurse meets these expectations.
  • Ensures that individuals and staff receive prompt, quality response to their medication questions.
  • Ensures appropriate processing and completion of requests for refills and consultation as authorized by the prescriber.
  • Communicates medication changes, medical, and psychiatric needs and updates to other treatment team members, both internally and externally.
  • Collaborates with administrative staff at programs to ensure that psychiatry staff has the resources and support needed and updated.
  • Communicates thoroughly and effectively with Program Director, Assistant Program Director, and other program supervisors/managers.
  • Collaborates with pharmacies to ensure that regular communication occurs with regard to prescriptions, prior authorization, medication packaging, sampling, and patient assistance programs.
  • Provides coverage in emergency situations. 
  • Performs other duties as assigned.

Qualifications:

Must have one of the following:

  • A valid Licensed Practical Nurse (LPN) license in the state of Pennsylvania and at least 2 years of experience; OR
  • A valid Registered Nurse (RN) license in the state of Pennsylvania and at least 1 year of experience. 

Must have all of the following:

  • Must be able to work flexible hours to meet individual's and program's needs.
  • Must have a valid driver's license, good driving record, and access to a reliable vehicle to attend trainings, meetings, and/or transport individual to appointments.
  • Must have the ability to obtain clearances as defined by regulations.

*Experience with behavioral health population preferred but not required.
